The Healthy Workplace Check is created for business decision-makers such as owners, general managers, work health and safety representatives, and human resource managers to reflect your understanding of policies, practices and programs currently existing in your workplace.

As such, the best person to complete this survey is the person who would be mainly, or at least partly, responsible for worker health, safety and wellbeing at your workplace.

This Healthy Workplace Check takes about 15 minutes. There is no right or wrong answers to these questions, simply answer to the best of your knowledge. The answers you provide show how much you agree with a particular statement through a scale of strongly disagree to strongly agree.

Based on your answers to the Healthy Workplace Check, a report will be generated which will provide you with tailored information about prevention and intervention strategies for your workplace.

You can choose to complete the Healthy Workplace Check anonymously or provide your workplace contact details. By providing your workplaces contact details, you might choose to be contacted by the Healthy Workplace Service to receive further tailored support. Further, if you choose to complete the Healthy Workplace Check again in the future, your results can be linked to any previous survey so you can compare your progress.
